Below you will find helpful forms and resources, including Back-to-School forms, Booster purchasing information, important dates, PowerSchool instructions, and 2023-24 tuition payment options. 
For your convenience, most of our back-to-school forms can be completed online through the Powerschool Parent Portal.  Physical exam forms and school picture forms should be completed using the links below.  New students will receive information for Powerschool login information prior to the start of school.  If you are a returning family and have trouble logging into your account, please refer to the video in PowerSchool instructions below.  If you still need assistance, please email [email protected].

The following forms are further explained below. Please print, complete, and return these forms to Kenny Klase, Director of Athletics. If you have any questions, please contact Kenny at [email protected] or (412) 561-3583 ext. 520.

 

1. All athletes are required to turn in a completed HIPPA form and PIAA packet to a coach or the athletic trainer before or on the start date of their initial athletic season.

2. Physical evaluations that were completed for school or other extracurricular activities will not be accepted for athletics; section 6 of the PIAA packet, "PIAA Comprehensive Initial Pre-Participation Physical Evaluation and Certification of Authorized Medical Examiner," will need to be completed by approved medical personal (found at the bottom of section 6) and submitted with PIAA packet.

3. All athletes participating in more than one sport will need to turn in section 7 "Re-Certification by Parent/Guardian," and additionally, if an injury occurred between sports seasons, section 8 "Re-Certification by Licensed Physician of Medicine or Osteopathic Medicine."

These forms also need to be completed and turned into a coach or the athletic trainer before, or on the start date of, additional sports seasons. Failure to submit the above-mentioned documentation on the designated season start dates will result in an athlete's inability to participate. An athlete will continue to be withheld from season practices until all of the forms are received by a coach or the athletic trainer.

If your child will miss more than 3 school days please direct them to fill out the extended absence form HERE or pick up a paper copy available in the main office. 
To access the PowerSchool mobile app, please follow the below instructions: 
  1. Download the "PowerSchool" app from your SmartPhone's app store
  2. Enter our district code: MQDD
  3. Enter your username and password.

Payment in Full by June 15, 2024: If you prefer to pay in full and not create a Tuition Management account, this payment must be received by June 15, 2023. 
 
Tuition Management 10-Month Payment Plan: The ten monthly payments will begin in July with the final payment due April 30, 2025. 529 Plans: Since 2018, tax-free distributions from 529 Plans can now be received by students enrolled in private elementary or high schools who pay tuition expenses. For more information, please see the PDF in the sidebar (mobile users: bottom of the page). 

Emails are being marked as Spam

Email notifications sent from SchoolMessenger can sometimes be mistaken as Spam, Junk, or Clutter by your email service provider. If you find SchoolMessenger emails are being marked as Spam, Junk, or Clutter, you can add the slshs.org and spe.schoolmessenger.com domains as trusted senders.

I would like to update my contact information

  1. Sign into your PowerSchool Parent Portal account.
  2. Select the Contact Information tab on the left navigation.
  3. Update information in the Home Phone, Cell Phone, or E-mail fields under Guardian 1 or Guardian 2.
  4. Once submitting changes, please allow 24 hours for changes to reflect in SchoolMessenger.

 

I am not receiving text messages

  1. Sign into your PowerSchool Parent Portal account.
  2. Select the Contact Information tab on the left navigation.
  3. Verify your mobile phone number is listed in the "Cell Phone" field. 
  4. Once verifying the above step, text "Y" to 67587 from this device.
  5. Sign into your SchoolMessenger account, and make sure "text me" is enabled for the alert type.

 

I am not receiving phone calls/emails

  1. Sign into your SchoolMessenger account.
  2. Verify your phone number or email address is enabled to receive alert messages by selecting the “call me” or “email me” options.
  3. If your email is enabled, check your spam or junk folders. Follow the tips under the above “Emails are marked as spam” heading to make this a trusted email.
  4. If your phone number or email is incorrect, please follow the steps in the above “I would like to update my contact information” heading.
